I the undersigned hereby personally promise and agree to guarantee payment to Osage Valley Electric Cooperative for all debts incurred by the business listed below. In the event of nonpayment of said debts, I the undersigned personally agree to pay all reasonable attorneys’ fees, costs of suit and collection fees. I further authorize Osage Valley Electric Cooperative to check my personal credit history at their discretion. I understand that the deposit that may be required will be based on my 3 month average usage and any such deposit will not be returned but will be applied to the final bill upon termination of the account with Osage Valley Electric Cooperative. I further authorize Osage Valley Electric Cooperative to disconnect for non-payment any other accounts that I may have with Osage Valley Electric Cooperative under the business listed below or in my name should this account go 30 days past due.
